From November 10-19th, 2017 faith communities across Waterloo Wellington are invited to participate in “It’s Time to Talk”; a campaign to talk about the vital importance of Conversations Worth Having with loved ones about wishes for care at the end of life.

Forest Hill United is pleased to be hosting a presentation about Advance Care Planning; Substitute Decision Making on Wednesday, November 15, 2017 from 7:00 p.m. to 8:30 p.m. Deb Snider, a Forest Hill United congregant and a volunteer with the Conversations Worth Having project will be the presenter.

In this presentation you will learn how to decide who will make future health care decisions for you if you are unable to do so. We will explore how to have conversations with your family and loved ones about your wishes, values and beliefs, and anything else that will help your Substitute Decision Maker to understand how you would like to be cared for in the event you are mentally incapable of making health care decisions for yourself. There will be time for coffee and questions following the presentation.

To ensure we have enough handouts, please confirm your attendance at embw@rogers.com.
